Hospital To Home Care: Care at Home for Post Hospitalization Older Adults
Is your loved one preparing for discharge from St. Joseph Regional Health Center, Memorial Hospital, Kindred Hospital, Unity Medical & Surgical Hospital, or Elkhart General and in need of care at home, help with showering, preparing meals or getting to doctor/therapy appointments?
Has your loved one recently been hospitalized for COPD, CHF, Diabetes, Emphysema, Glaucoma, Heart Disease, Stroke, Pneumonia or some other illness? Recently had surgery or a fall and then physical therapy?
We can ease your or your loved one’s transition from the hospital or rehabilitation facility to home, which can lead to a better, more comfortable recovery.
The time immediately after a hospital stay can be critical to the health and recovery of a loved one. Studies show that the lack of proper support and care at home following surgery or a serious hospitalization can lead to slower recovery or even readmission to the hospital.
Family members often take on the task of caring for a loved one after a hospital stay but can find they are untrained or ill prepared to meet the physical and emotional challenges of such care. That is why Comfort Keepers® offers services to ease the transition home.
What is Transitioning Care?
This specialized care at home helps ease your loved one’s transition from a hospital stay. Some of the services Comfort Keepers can provide includes the following:

How Comfort Keepers Can Help
In addition to the customary homemaker/companion and personal care at home that Comfort Keepers provides, we are able to adapt services to accommodate clients with specific medical needs such as
- congestive heart failure.
- myocardial infarction (heart attack).
Let Comfort Keepers help guide you safely through the transition process. It offers clients peace of mind and can help with the recovery once your loved one returns home. We are available 24 hours a day, seven days a week to help your family member recover.
In addition to in-home services Comfort Keepers offers to post hospitalized patients, consider a Safety Choice product from Comfort Keepers.
Safety Choice by Comfort Keepers offers the traditional medical alarm, the fall detection pendant, the wireless medical alarm, medication dispensing units and other products.
Traditional medical alarms are the devices whereby a post hospitalized patient would wear a waterproof pendant and in the event of a fall or medical issue that needed attention could push the pendant and activate the console connect to the landline. The activation of the console results in live responder addressing client through speaker on machine asking “Do you need help? “ To which the client may respond “yes”. The live responder would then take the appropriate action either calling family member or an ambulance.
Some of the other products offer similar services but have additional uses such as fall detection pendant. The pendant works same as traditional medical alarm with the additional technology of activating on its own if loved one falls. No pushing of button is needed.
